With group health insurance all employees who enroll are covered by a single policy. The premium for group insurance is based on number of employees enrolling, average age, average sex, SIC code and zip code.

Nevada and Arizona, like most states, have state regulations that a small business with at least two W-2 employees may apply for "guaranteed" group health insurance coverage. This is the major advantage of group health insurance over individual or family insurance. Without "guaranteed" coverage employees would need to apply for individual health insurance and may be penalized with pre-existing conditions or declined coverage.

Companies looking for employee benefits will fall into one of three group classifications; small group, mid-size group, or large group. Each group is regulated by different state statutes and different underwriting rules.

Small Health Insurance Group
The size classification that determines a "small business" in Nevada and Arizona is a business between 2 and 50 employees. Small group health insurance is guaranteed issue and if the company meets the group health insurance requirements they cannot be turned down for coverage.

Mid Size Health Insurance Group
Mid-size groups are 50 plus employees and depending on the carrier could be as large as 100 or 200 employees. Unlike small group, mid size is not guaranteed issue and works on an "offer" and "acceptance" process. The carrier looks at the group enrollment and makes a rate "offer" to the employer. The employer can "accept" or "decline" the offer from the carrier.

Large Health Insurance Group
Large groups typically have 200 or more employees. Like mid-size, large group health insurance is not guaranteed issue but with larger groups the risk to the insurance company is spread out and the "offer" is typically cost effective. The main advantage of large group plans is the flexibility of plans and the opportunity for fully or partially self insured plan designs.
